Abstract
El Trastorno Obsesivo-Compulsivo (TOC) en la infancia y la adolescencia ha recibido poca
atención en la literatura científica. Sin embargo, se estima que hasta un 80% de los adultos
con TOC lo han padecido desde antes de los 18 años, a menudo incluso a edades tan
tempranas como los 6 años de edad. Uno de los factores que más parece relacionarse con la
gravedad del TOC infanto-juvenil es la Acomodación Familiar (AF). La AF comprende
aquellos comportamientos realizados por los familiares con el objetivo de reducir el malestar
del niño, y se manifiesta tanto mediante conductas de evitación de situaciones ansiógenas,
como de participación en los síntomas. Este trabajo busca sistematizar las variables
asociadas a la AF realizada por los padres/cuidadores primarios y su relación con el TOC,
así como qué variables de los niños y los progenitores se relacionan con la AF. Tras la
revisión de 25 artículos los principales hallazgos son: (1) La AF realizada por los padres de
niños con TOC es elevada; (2) la AF y la gravedad del TOC se afectan bidireccionalmente;
(3) las variables familiares, los síntomas externalizantes en el niño, y las emociones
difíciles de los padres correlacionan con la AF; (4) la AF predice el deterioro funcional
asociado al TOC y tiene un papel mediador entre este y la gravedad del TOC. Estos
resultados son útiles de cara a perfeccionar tratamientos futuros, especialmente en relación
con aquellos casos resistentes a los tratamientos actuales
Obsessive-Compulsive Disorder (OCD) in childhood and adolescence has received little attention. However, it is estimated that up to 80% of adults with OCD had its onset before they were 18 years old, and often as young as 6 years old. One of the main factors related to the severity of pediatric OCD is Family Accommodation (FA). FA refers to a group of behaviors carried out by family members with the aim of reducing the discomfort of the child, and it is performed through facilitating the avoidance of anxiety-inducing situations, and by participating in the child's symptoms. This review aims to systematize the variables associated with FA performed by parents/main caregivers and its relationship with OCD, and to analyze which variables of children and parents are related to FA. After reviewing 25 articles, the main findings are: (1) FA is high in parents of children with OCD; (2) FA and OCD severity affect each other; (3) family variables, child externalizing symptoms, and difficult parental emotions correlate with FA; (4) FA predicts OCD related functional impairment and has a mediating role between functional impairment and OCD severity. These results are a useful tool to improve future treatments, especially for children resistant to the current ones
